Medical Clearance Requirements and Insurance Information

MEDICATION EXEMPTION FOR BANNED SUBSTANCES FORM  To check if you are taking a medication that is banned by the NCAA, please click here. You will need to sign up for an account to look up your medication. The organization is NCAA Division III and the password is ncaa3. IF YOUR MEDICATION IS BANNED, YOU WILL NEED THIS FORM. Give this form to the prescribing physician for them to complete. This needs to be completed and uploaded before you can compete.



MEDICAL INSURANCE REQUIREMENTS All students enrolled at UC Santa Cruz are required to carry their own primary insurance either through the campus' UC SHIP insurance plan or a different health insurance plan. More information regarding primary insurance and secondary insurance can be found in the medical clearance instructions above.

If you decide to carry your own there are two things you need to pay attention to.

  1. Opt out of UC SHIP - click on this link "UC SHIP FAQs" to learn how to do this. There are deadlines to opt out so read this carefully so you do not miss them. 
  2. Consider signing up for CruzCare. For a fee, CruzCare allows the student to access the on-campus Student Health Center for basic injuries and illnesses which is convenient. Click on this link CruzCare FAQs for more information.
